I am not under the influence of drugs or alcohol.

I give permission to yaya to take/use photos of my tattoo before, during, and after each session.

I acknowledge that improper care of my tattoo can result in infection, scarring, or ink fall out. I acknowledge I have received aftercare instructions and I agree to follow them while my tattoo is healing.

If 30% or more of the original design is changed or there are more additions to the design by you(the client’s) request, on the day of the appointment there may be additional fees and/or rescheduling due to the account of impromptu extra drawing time subjected by the client.

The estimate provided by the artist is an estimated time quote and I acknowledge that tattooing time and price may vary.

I acknowledge I am over the age of eighteen and have truthfully represented my age, and that obtaining a tattoo is my choice alone.

I agree to release and forever discharge and forever hold Yaya Ink from any and all damage, claims, or legal actions arising from or connected in any way with my tattoo or procedures and conduct used to apply my tattoo and any and all tattoos applied by Yaya in the future.

I acknowledge and agree to any and all tattoo instructions for proper after care of my tattoo and failing to do so will result in future touch-ups at my own expense.

I agree to contact Yaya within a 1 year period following the start date of my tattoo for any touch-up service. After the 1 year, I acknowledge I forfeit a complementary touch-up by Yaya and will have to schedule a normal tattoo session at my own expense
